General Instructions

1. Patients under the age of 18 must be accompanied by a parent or legal guardian. Please do not drop off minors as you will need to sign consent forms, and discuss the nature of the procedure.

2. Please bring your referral forms with you from your dentist.

3. For patients having general anesthesia or sedation (going to sleep):

Do not have anything to eat or drink (including water) for six (6) hours before your appointment (TAKE YOUR REGULAR MEDICATION WITH A SMALL SIP OF WATER).
A responsible adult must accompany you to the office, remain at the office during surgery, and drive you home.
Do NOT drive a vehicle or operate any machinery for a minimum of 12 hours following general anesthesia or sedation, or longer if you feel any residual effect of the anesthetic.
Please wear loose fitting clothing with sleeves that can be rolled past the elbow. Dentures and contact lenses will be removed prior to surgery. If possible, please wear you glasses instead of contact lenses.
Bring your medications with you and a list of changes.
